Below are some instructions on how to apply through some local Universities that we've dealt with recently:

Weber State University

1.  Call 801-626-7004

2.  Tell them you are a teacher and would like to have

      your Perkins Loan Forgiven

3.  They will send the form to you to fill out and return

4.  Send completed form to:  loans@weber.edu
